District 11 Athletics Athletic Physical Information 2025-2026

D11 Middle and
High School Families, 

CISM Colorado Institute of Sports Medicine will be offering $10 sports physicals on a first-come come first-served basis on Tuesday, July 22, from 4:00 - 6:30.

CISM is located at 1625 Medical Center Point. Ste. 180. Please enter CISM from the South side of the building. Sports Physicals will be on hand, and payment can be made via credit card or GoFan.

El CISM Colorado Institute of Sports Medicine ofrecerá exámenes físicos deportivos de $10 por orden de llegada el martes 22 de julio, de 4:00 a 6:30.

El CISM se encuentra en Medical Center Point 1625, Ste. 180. Ingrese al CISM por el lado sur del edificio. Se realizarán exámenes físicos deportivos y el pago se puede realizar con tarjeta de crédito o GoFan.
